1.    Has your worksite/facility developed policies and procedures regarding H1N1 Flu?

 

 

 

2.    Have you received specific training from your employer regarding H1N1 flu ?

 

 

 

3.    Have you been vaccinated for “regular flu”?

 

 

 

4.    Do you plan to be vaccinated for H1N1 flu?

 

 

 

5.    Does your employer provide N95 respirators (not surgical or procedure masks) for protection against transmission of H1N1 flu from known or suspected patients?

 

 

 

6.    Have you been fit tested to your respirator model and size in the past year?

 

 

 

7.    Did your fit test take longer than 10 minutes to complete?

 

 

 

8.    Has your worksite/facility told you which procedures are considered high risk for your exposure to H1N1 flu when done on known or suspected H1N1 patients?

 

 

 

9.    Is your worksite/facility modifying normal procedures to better deal with the upcoming H1N1 flu season

 

 

 

10. Has your worksite/facility modified their absentee policies to encourage staff sick with Influenza-like illness (ILI) to stay home until they recover?
